Restrictions:
- Requires a valid prescription. You can either mail the prescription to us, or we can contact your doctor or pharmacist to request a prescription. Your prescription will be filled for the quantity prescribed by your doctor.
- Applicable rebate will be issued within 2 weeks after the order has been shipped.
- Customers with insurance will receive a rebate, or their out-of-pocket co-pay, whichever is less, on the same credit card that was used to place the order.
